Minors participating in GVC-ADK activities

  • Minors are welcome to participate in GVC-ADK activities; however the minor (any person under 18 years of age) must be accompanied by a parent or legal guardian.
  • If that is not the case, the minor must be accompanied by a responsible adult who must provide both a Release of Liability Minor form and an Emergency Medical Care form signed by a parent or legal guardian.
  • These forms are available for downloading at the bottom of this page.

Liability Release (Waiver) Form

  • January 26, 2011 - New Liability Release Form - We have a new liability release / trip sign-up form incorporated new text from main club. Please use the new form for all trips.
  • July 24, 2009 - added line numbers
  • March 21, 2009 - added the chapter PO box
  • February 12, 2009 - updated form submission instructions
  • December 1, 2004 - major revision -- previous versions are obsolete
  • November 12, 2002 - incorporated release for minors
